Why we do this

Coming home is hard.

The system doesn't make it easy to see a doctor, get your meds, or land a job. And most people are expected to figure it all out within weeks of release.

We don't think that's right. The Fresh Start Health Initiative exists to make sure that when you walk out, you're not walking out alone.

Your health is the foundation everything else gets built on, so that's where we start.

How it works

Four steps. We do most of them with you.

    Fill out the form, or call us.

    Whatever's easier. Both go to the same place, and both get a real person.

    We reach out within 1–2 business days.

    An advocate calls to talk through what you need most right now.

  3. 03

    We get you enrolled in healthcare.

    At no cost. We do the paperwork; you tell us when you're free for appointments.

  4. 04

    You pick up your shoes. We keep working.

    ID, resume, job training, housing connections, whatever's next.

Workforce development and job placement. They help you turn the page back into a paycheck.

Grace Community Outreach

Community support and reentry services. Boots-on-the-ground connections to housing, food, and transportation.
